Why baking soda and baking powder aren't interchangeable 1:1
Baking powder is essentially baking soda pre-mixed with a powdered acid and a cornstarch buffer, so it reacts on its own once it hits liquid. Baking soda is just the base ingredient, roughly three to four times stronger by volume, which is why a full teaspoon of baking powder only needs about a quarter teaspoon of baking soda to match its leavening power.

The formula used
Converting baking powder to baking soda divides by the standard 4 to 1 ratio.
The acid needed scales with the resulting soda amount, at roughly double the volume for cream of tartar, lemon juice, or vinegar. Converting the other direction simply reverses the ratio, since baking powder needs no added acid.
Choosing the right acid for your recipe
Cream of tartar is the safest default, since it has no flavor of its own and adds no extra liquid to the batter. Lemon juice and vinegar are close seconds and are common enough that most kitchens already have one on hand, though both add a small amount of extra liquid you may want to account for in a delicate recipe.
Buttermilk and yogurt work too, but they call for a much larger volume relative to the soda, so they're best used when a recipe already includes a liquid dairy ingredient you can simply swap rather than adding on top of everything else.
Going from soda to powder is the easier direction
Since baking powder already contains its own acid, replacing baking soda with baking powder doesn't require adding anything extra, just roughly four times the amount by volume. The one thing worth noting is that many soda-based recipes also include an acidic ingredient like buttermilk for flavor and moisture; that ingredient is still fine to keep, it simply stops being chemically necessary once baking powder is doing the leavening.
Keep in mind that most store-bought baking powder is double-acting, reacting once when mixed and again with oven heat, while baking soda reacts immediately on contact with acid, so a soda-based batter should generally go into the oven promptly.
A full worked example
Say your recipe calls for 2 teaspoons of baking powder and you only have baking soda and lemon juice in the kitchen. Dividing 2 teaspoons by 4 gives 0.5 teaspoons, or half a teaspoon, of baking soda needed.
For the acid, lemon juice is used at roughly double the soda amount, so 0.5 times 2 gives 1 teaspoon of lemon juice. In grams, that half teaspoon of baking soda works out to about 2.3 grams, using the standard 4.6 grams per teaspoon reference weight.
Going the other direction, if a recipe calls for 3/4 teaspoon of baking soda and you only have baking powder, multiply by 4: 0.75 times 4 gives 3 teaspoons, or 1 tablespoon, of baking powder, with no additional acid required.
